Simple answer - the clutch plates wear down and lose their grabbing ability. Every time you pull the clutch lever, the clutch plates are slipping and sliding against each other. you can add to the other answers answer-someone riding the clutch lever repeatedly way too long. Clutches can slip because the cable is out of adjustment, the internals get weak or worn and because of neglect or abuse of the owner or rider, no matter if intentional or not.
